This form takes about 10 to 15 minutes. Every question exists to help us find the best match for a specific dog, not to screen people out. Tell us about your home, your household, and the kind of dog you’re hoping to bring into it. You’ll need to be 21 or older, and if you rent, you’ll need your landlord’s written permission.
Please Note:
We only contact applicants who are chosen as the most suitable match for the dog and their unique needs. We will keep your application on file for 3 months. If you are interested in another dog, contact us to update your current application — please do not reapply.
